What to bring to your first appointment

We want to ensure that when you come to see us we can focus on you, not on administrative matters. So when you come to see us you should bring:

• A copy of Medicare Care Plan and any referral letters you have been given.
  
• Claim number or approval letters given to you by an insurance company.
  
• Your payment, as discussed at the time you made your appointment.
• Anything else you feel may be relevant, such as relevant report, letters, etc.
